Scheduling an Appointment
If this is your first appointment with the Health Center, you MUST schedule a New Patient Visit (40 Minute Appointment). New patients who schedule other visits, will be asked to reschedule.
If you are a new patient, please make sure to arrive at least 10 minutes prior to your appointment time to complete required paperwork OR download and complete the required paperwork prior to your appointment by clicking here.
NOTE: All patients requesting vaccines, must have been seen at the Health Center within the last year. If you are scheduling an appointment for a vaccine, you are required to bring all childhood or appropriate adult vaccine records with you to your appointment.
To accommodate all patients requesting to be seen in the clinic we ask that you please be on time for your appointment. If you are more than 10 minutes late for an appointment you may be asked to reschedule.
